Investigation Summary
At approximately 12:00 p.m. on September 30, 2007, Employee #1 was inspecting chips in the attempt to assure quality snacks in packaging. Employee #1 was walking towards the machine and she did she stepped on a single Tostitos chip on floor. Employee #1's foot slid under her causing her to fall onto the cement floor. Employee #1 suffered a fractured left ankle. Employee #1 was treated but was not hospitalized.
