Investigation Summary
On December 28, 2006, Employee #1 was attempting to repair an orchard sprayer. When he pulled on the belt, it activated the sprayer and pulled his finger in between the belt and the pulley wheel, amputating his right index finger at the first knuckle. Employee #1 did not deenergize the sprayer before attempting the repair. He was hospitalized for his injury.
