Event: 12/24/2006
Employee Is Killed in Utility Vehicle after Brakes Fail

At approximately 9:00 a.m. on December 24, 2006, Employee #1, working as a gardener for Rea Ranch, entered a Gator utility vehicle that was on a grade of approximately 10 to 30 degrees. When he removed the parking brake, the brakes failed and the vehicle rolled down a winding driveway. He crashed into a pole about 157 feet down the driveway and was killed. Further investigation revealed that the vehicle was not maintained in accordance with the manufacturer's instructions and was not checked prior to each work shift as required. In addition, it was determined that the brakes failed due to the loss of a cable pin.
