Investigation Summary
On September 8, 2006, Employee #1 was riding on a road widener that was being operated by a coworker. The drive shaft of the widener fell to the pavement below and the coworker lost control of the vehicle. Employee #1 jumped from the moving widener and struck his head on rocks and pavement. Employee #1 was transported by helicopter to a local hospital where he died.
