Employee Is Killed When Run over by Forklift

On June 6, 2006, Employee #1, while assembling concrete tunnel formwork, stepped off a 1-foot high platform just as a rough terrain forklift was passing by. Employee #1's foot caught under the back tire of the forklift, and it ran over him. The driver stopped the forklift and backed off Employee #1. Employee #1 died at the scene from his injuries.
