Investigation Summary

Investigation Nr: 201128154
Event: 04/08/2011
Employee's Fingers Are Injured in Timing Belt Sprocket

At approximately 2:21 p.m. on April 8, 2011, Employee #1 was a brake, oil and lube technician at Phil's Tire and Auto. A mechanic coworker had completed work on a Mitsubishi Lancer engine involving replacement of the head and timing belt. The belt was not tracking properly in the sprocket assembly and was making a noise. The coworker asked another mechanic to visually check the belt with the car hood open and the cover for the timing belt removed. One mechanic started and stopped the engine a few times as the two mechanics discussed the issue. Employee #1 went to the car to see what was going on and was on standing on the passenger side of the engine compartment. Neither mechanic had asked for assistance from Employee #1. With one of the mechanics sitting in the driver's seat, Employee #1 reached into the engine compartment and was testing the timing belt tension with his right hand. When the mechanic started the engine, Employee #1's thumb and forefinger were pulled around the sprocket. He was treated for a partial amputation and for lacerations to his thumb and finger and for a tendon injury.
