Investigation Summary
At approximately 7:00 a.m. on December 8, 2009, Employee #1 was using a power washer on a roof. Employee #1 was on a single-story building, with a sloped roof, approximately 12 feet high. He was wearing a harness, and the lanyard was not tied off. Employee #1 stated that when he went on the roof, he realized that lanyard was not long enough to perform the job. Employee #1 was trying to get off the roof to get a new lanyard, when his foot slipped and fell off and landed on a cement floor on his back, seriously injuring his lower back. Employee # 1 was hospitalized for three days and required surgery. Employee # 1 worked for K N Properties.
