Investigation Summary
On October 19, 2009, Employee #1, a foreman, and a coworker were unloading scaffolding. Forty-two scaffolding-frames, weighing approximately 35 pounds each, were stacked up on an open flat-bed truck, standing in a vertical position. Employee #1 parked this truck on an incline, approximately a 13 percent grade, with the back of the truck facing downhill. The truck was 4 ft above the ground. The coworker was wearing a harness, but had no where to tie off. Employee #1 and the coworker climbed up on the truck-bed to unload the material. They were on the stacks, approximately 2 ft high. The securing strap on the scaffolding-frames was released. The coworker stood up on the side of the truck-bed next to the stack of scaffolds, and Employee #1 stood up on a pile of wood planks which were in front of the scaffolding frames. They began sorting out the frames, but they became unstable and shifted towards them. They were knocked off backwards and fell off the truck; approximately 6 ft. Employee #1 fractured his neck and was hospitalized.
