Investigation Summary
At approximately 11:30 a.m. on May 8, 2009, Employee #1 was cleaning or scraping chocolate from a chocolate molding machine ("Greer Hopper -1538 -WE") that had not been deenergized or locked out. The agitator, a moving part that rotates to mix the chocolate, was disengaged by pulling out a lever, but it was not mechanically blocked or locked out to prevent inadvertent movement during cleaning. When Employee #1 was scraping down the hopper, the agitator inadvertently moved and hit her right hand. Employee #1's right hand was severely lacerated, and she was bruised from her hand to her elbow. Employee #1 was taken to Peninsula Hospital and released after four hours. The fire department reported the accident to the San Mateo District Office of DOSH. The employer was cited for lockout/tagout violations.
