Employee Fractures Leg and Ribs in Fall from Platform

At approximately 10:00 p.m. on July 5, 2007, a coworker was working on the Platform H-21, when he accidentally dropped the tape-measurement, solid square piece having a small ring on the corner to hold, into a tank which was approximately 9 feet deep where chocolate was melting. The coworker did not have any appropriate tool to pull out this object, so he got a 7-8-ft long and 0.125-in. wide metal rod from the maintenance shop and bent the one end of the rod to shape in a hook to fish out the object. When the coworker was still struggling to pull this object out from the tank, he asked an employee for help who was passing by. The employee went up on the platform to pull out the tape with this hook. While the employee was pulling out this object from the tank, he started walking backwards and fell from the platform. The employee landed on the concrete floor from the height of 7 feet 2 inches. The employee was hospitalized for six days for fractured femur and two ribs. He also had cut on his head.
