Investigation Summary
At approximately 12:54 p.m. on May 9, 2007, Employee #1, a truck driver, was injured while storing the forks of a Caterpillar, Model Number R-50 Rough-Terrain Forklift, and Serial Number 2PJ00431 into their traveling position. Employee #1 completed a delivery of lumber and hitched the forklift to the flatbed truck and stowed the left fork in its travel position by tilting it back on the pivot point of the support bar. While attempting to stow the right fork Employee #1 apparently lost solid footing from gravel on the sloped roadway and was unable to complete the lift. The tow bar fell back into its working position and caught Employee #1's gloved right hand between the back of the fork upright section and the support bar. Employee #1 suffered injuries to his right middle finger including bone fractures of the first digit, lacerations and disturbance of the nail and nail bed. Employee #1 was taken by ambulance to the hospital emergency room where he was treated and released approximately seven hours later.
