Investigation Summary
On May 7, 2007, Employee #1 was delegated to retrieve material that was approximately 13 ft above ground level and up on shelves. The employee elevated himself on a Crown Stockpicker in order to acquire the material. At the time, no pallet was stationed on the forks of the Crown Stockpicker while in operation. This created a floor opening on the Crown Stockpicker platform. Employee #1 was not tide off with an operator harness. The employee fell from the platform approximately 13 ft onto steel railing and concrete. The employer failed to ensure that Employee #1 was wearing approved personal fall protection equipment. The employer failed to guard every floor opening with a cover and guardrails on all open sides. The employer failed to ensure that every employee utilized personal fall protection equipment. Employee #1 suffered a head fracture. The employee was hospitalized.
