Investigation Summary

Investigation Nr: 201126083
Event: 01/03/2007
Employee Is Injured in Fall from Truck

At approximately 9:30 a.m. on January 3, 2007, an employee, the driver of a Peterbilt 10- wheeler dump truck, pulled into the Andreini Brothers yard and parked in a reloading area. A coworker witnessed him fall as he tried to exit the vehicle. The truck was fitted with a steel grab bar located just outside and to the immediate rear of the drivers cab area. Apparently, the employee exited the cab, he reached to grip the grab bar with his left hand, lost his footing on the first step landing, and his left hand lost grip on the grab bar. He fell approximately 31 inches to the ground, striking his tailbone on one or both of the steps. The Paramedics took the employee to Seton Medical Center, Daly City, where he remained until his release on January 17, 2007. He incurred injuries to his spine, discs, left shoulder and rotator cuff.
