Investigation Summary
On May 30, 2007, an employee removed the guard and was unjamming the Sealant Machine. The V notch slide activated and her finger got caught between the hole of the nut plate and stationary bar. The residual energy was not restrained. The right middle finger to the first knuckle was amputated. The employee was hospitalized and treated for finger amputation.
