Investigation Summary
On January 30, 2007, Employee #1 was helping Employee #2 unload a stack of flat cardboard boxes from a Toyota forklift truck. For some unknown reason the boxes would not move off the forks, so Employee #3 who was in the area decided to help. He put his right hand on the stack of boxes and his left hand on the mast of the machine. The forklift operator used the side shifter, and Employee #3's fingers became caught in the forklift. Employee #3 was hospitalized. His index, middle, and ring fingers were amputated to the first digit.
