Investigation Summary
Employees were removing a 90-ft long, 3000-pound metal grain transfer spout. The spout was suspended by a crane and synthetic sling. Employee #1 was working from a fixed, elevated platform. When Employee #1 disconnected the upper end of the spout, the spout pivoted and struck the platform. The platform failed and caused Employee #1 to fall approximately 100 ft to a lower level. Employee #1 died from his injuries.
