Investigation Summary
On October 12, 2006, Employee #1 was participating in a dredging operation on a lake. His assignment was to attach a tie line between a tugboat and a dredge. He leaped from the dredge to the tugboat, when he slipped and fell into the lake without a life jacket. A coworker tried to rescue him, but failed. Employee #1 was not able to stay afloat and drowned.
