Investigation Summary
On September 7, 2006, Employee #1 observed material build-up in the M-13 cyclone while performing a plant micro clean-up. Employee #1 put his hand inside to get material out with the feeder running and not locked out. The blade of the feeder caught and severed Employee #1's right hand above the wrist and he was hospitalized.
