Investigation Summary
On September 1, 2006, Employee #1 was on the flat roof of a school entryway that was approximately 19 ft above an asphalt sidewalk. While looking for the extent of damage to a roof perimeter copping stone, Employee #1 leaned against the stone. The stone moved, causing Employee #1 to fall to the sidewalk. Employee #1 struck his head and died. No fall protection was being used.
