Employee Is Killed When Crushed in Paper Roll Machine

At approximately 12:15 p.m. on May 31, 2006, Employee #1, an assistant operator of a paper roll rewinder machine, entered the machine to staple the new run of paper to the cores on the mandrel. A second employee, the operator, was at the operator's control station. A third employee, a trainee, was at the garbage bin disposing of excess paper. An L-shaped table is located at the exit of the machine and used to discharge the completed paper rolls onto the floor. The table can be placed in the "up" or "down" position. The table was positioned midway between the "up" and "down" interlocks, and a gate above the table was in the partially "down" position, between interlocks, also. Employee #1 started to staple the paper from the drive side on core Number four. He was located in the machine between the drum and the table. He had his back turned to the table. The table went to the "down" position, striking Employee #1 and crushing him at the waist against the rolls. The operator witnessed the table lower and attempted to raise the table off of the victim with no success. A fourth employee ran over and raised the gate to meet the interlock, allowing the table to be raised. Employee #1 was transported to Washington County Hospital, where he died at approximately 2:45 p.m
