Employee Is Killed by Falling Scaffold Plank

As Employee #1 was erecting a tubular welded-frame scaffold on a construction site, he fell approximately 22 feet to the ground. Employee #1 was then struck on the head by a falling 2-in. by 10-in. by 12-ft scaffolding plank and killed.
