Investigation Summary
June 26, 2006, Employee #1, of Tire Distribution Systems Inc., was dispatched to a road project on Hwy 18 to change a flat tire of a dump truck. Employee #1 had lifted the truck with a hydraulic jack under the right rear axle, removed both tires, removed the flat tire from the wheel, and placed it into the back of the service truck. Employee #1 was later found by a service manager lying on his back with head trauma with the old wheel and new tire lying across his feet. Employee #1 was pronounced dead at the scene.
