Event: 07/21/2008
Employee Fractures Ankle in Fall on Conveyor Line

At 3:40 p.m. on July 21, 2008, Employee #1, a manual bagger operator, was helping a second employee consolidate a partial pallet of cases from the roller conveyor onto another partial pallet on the floor next to the rollers. As Employee #1 was reaching for the last case on the roller conveyor, the tip of his right shoe got caught on the forklift wheel stop bar plate installed on the floor in front of the roller conveyor, causing him to fall and twist his ankle. Employee #1 was transported to St. Rose Hospital in Hayward where he was treated for a fracture to his right ankle.
