Investigation Summary

Investigation Nr: 201507852
Event: 08/17/2007
Employee's Finger Is Caught Between Chain and Sprocket

On August 17, 2007, Employee #1, a utility worker, was stacking and feeding flat sheets of corrugated board into a Staley jumbo box line prefeeder. It was at the final process, in which the stock had finished discharging into the machine, and the prefeeder table was descending with the wooden pallet to be discharged. The pallet was not rolling forward through the discharge chute, from the scissor-lift table. The employee went around the guarded fenced area to the feed side, while the machine was still in operation. With his right hand, he used a push broom to push the pallet, and held onto the alligator track above for leverage with his left hand. He had stepped in front of the photo-eye causing the prefeeder to activate the discharge while his hand was hanging on the top of the alligator track. His finger was caught between the chain and sprocket gear. His left ring finger was amputated at the first knuckle. Employee #1 was not hospitalized but had to see a hand specialist after the accident.
