Employee Crushes Finger While Demonstrating Machine

On May 10, 2007, Employee #1 was conducting a practical demonstration as part of training on the Bausch-Stroebel tray loader. Employee #1 was an experienced operator and a trainer. Employee #1 placed her right finger near the right edge of the pusher bar blocking sensor Number 1 and simultaneously another employee placed his hand to block sensor Number 2. Employee #1 did not give proper instructions that were readily understood by the employee that blocked the sensor Number 2. This dual action triggered the pusher bar to sweep and strike the tip of the Employee #1's right ring finger, crushing it against the conveyor guardrail. Employee #1 sustained an open fracture of the right hand fourth finger distal phalanx followed by surgical amputation. Employee #1 was admitted, treated and discharged on May 10, 2007. However on May 23, 2007, the same finger distal phalanx was amputated.
