Investigation Summary

Investigation Nr: 201507720
Event: 06/05/2007
Employee Fractures Leg in Fall

On June 5, 2007, Employee #1 was undergoing training as a cable-TV installer. Employee #1 was assigned to field work under two, more experienced, coworkers to install cable-TV for new customers. This assignment entailed climbing electric utility poles for installation. While attempting to climb an electric utility pole, Employee #1 became fatigued and fell 25 feet to a concrete sidewalk. He was taken to a nearby hospital, where he was hospitalized and underwent surgery to repair a fractured femur and tibia on his right leg. The accident investigation revealed that Employee #1 was overweight, which provided a challenge while climbing utility poles to install the cable-TV. Due to this added difficulty, Employee #1 became fatigued and was no longer able to adequately hold himself up, leading to his fall.
