Employee Is Injured in Fall from Plastic Bucket

On May 7, 2007, Employee #1 climbed a five gallon empty plastic bucket in an attempt to access a workbench. The employer provided ladders but they were not used. He placed his left foot on the bucket and lost his balance and fell approximately 15-in. He fractured his wrist and was hospitalized.
