Investigation Summary
On February 8, 2007, Employee #1 was working as an electrician for an electrical contractor. He was climbing a fixed ladder to access the roof of a building. Apparently his right hand lost its grip as he was holding the 16th rung. He pushed himself off from the ladder and fell from a height of approximately 10 feet to grade. He fractured the heel in his left foot, and he was taken to Eden Medical Center in Castro Valley, CA. He was admitted on February 8, 2007, treated, and discharged on February 11, 2007. Employee #1 had been climbing various kinds of ladders for last several years. This particular ladder had no problems, and he had climbed the same ladder many times before. The weather was dry. It was not raining. The ladder was new and in good condition. The rungs were not slippery. Employee #1 was not taking any kind of medication.
