Investigation Summary
On December 21, 2006, Employee # 1 was asked by the supervisor to operate the chip line (# S-3912), a routine job assignment. Part of the process involves using an extension tool to manually push the built-up masa (dough) into rollers. To reach the far side of the rollers, Employee #1 stood on an electrical junction box. As she stepped down from the box, her right foot slipped. In an attempt to balance herself she caught hold of a guard and her right hand slipped down about 24 inches toward the rear of a partially guarded sprocket assembly. She sustained distal phalanx amputation of her right index finger. Employee #1 was admitted and discharged from St. Rose Hospital on the same day as the injury. She confirmed that she had been trained to operate the machine. The investigation concluded that the employer did not provide employees with proper access to the far side of the rollers to push the masa dough.
