Employee Is Killed in Fall from Building Under Construction

On March 30, 2009, Employee #1 was involved in bolt-up operations of a building under construction. He climbed a fiberglass ladder and walked across a 16 inch wide concrete foundation wall. Employee #1 then stepped on a 6 inch flange of a filler I-beam, which shifted under his foot and caused Employee #1 to lose his balance. Employee #1 fell 21 feet toward the exterior of the building and struck the ground. Two coworkers witnessed the accident and contacted emergency services. Employee #1 was taken to a nearby hospital, where he later died after undergoing emergency surgery.
