Employee Is Crushed by Falling Scaffold Frames

On March 21, 2006, several employees were engaged in repairing scaffold frames that had been returned from a construction site, with minor damage. In order to repair the frames, a bundle of 25 frames was brought into the repair shop area, and placed against the wall. The frames were then removed, one at a time, to do the necessary repairs. Employee #1 was walking past the bundle of scaffold frames when they began to fall. He was pushed to the floor by them. Eighteen frames cascaded on him, crushing him between the frames and the floor. He died as a result of his injuries.
