Investigation Summary
At approximately 11:14 p.m. on January 9, 2007, a welder fell 15 ft 4 in. from the deck of a raw mill foundation located at the Hanson Permanente Cement, Inc., in Cupertino. The welder was not wearing his fall protection equipment. Portions of the deck guardrail were temporarily removed for access of a trunnion installation. The supervisor was on the deck supervising and instructing the crew. The welder was positioning a hydraulic jack in front of the trunnion. The welder briefly left the meeting to cut some steel plates to align the pedestal. When the welder returned to the meeting, he stood to the right and slightly to the rear of the supervisor. As the meeting concluded, the welder stepped behind the supervisor to obtain an additional hydraulic jack. At the same time that the welder stepped behind the supervisor, the supervisor also stepped backward to leave the area. The welder stepped to the side to avoid bumping into the supervisor, which caused the welder to lose his balance and fall to the concrete floor below. He was hospitalized with a fractured ankle.
