Employee Receives a Chest Contusion, Later Dies

At approximately 10:17 a.m. on September 15, 2009, Employee #1 fell against a conference room table and received a chest contusion. Employee #1 refused medical treatment. On September 20, 2009, Employee #1 reportedly suffered a fall at her home and was hospitalized. Employee #1 died of renal failure.
