Employee Is Cut by Box Cutter, Later Dies

At approximately 10:00 a.m. on April 1, 2009, Employee #1 was working in the shipping department as a shipper and packer. Employee #1 was cutting box blanks to ship an electronic component when the box cutter slipped and he stabbed himself in the right thigh above the knee. Employee #1 suffered a moderate laceration to the upper leg. Employee #1 refused help from his manager and a friend on the shipping line and an offer to be taken to the hospital by his supervisor. For the next four days Employee #1 came to work for shorter times each day. Employee #1 refused treatment. Employee #1 became progressively sicker. On the fifth day, neither his friend nor his employer could reach Employee #1 by phone. On the sixth day, Employee #1 was found not breathing on the floor of his home by family members. Medical services transported Employee #1 to the hospital emergency room on April 6, 2009 where efforts to revive him failed. At approximately 10:36 p.m. Employee #1 was pronounced dead.
