Employee Is Burned in Flash Fire

At 11:17 a.m. on May 7, 2007, Employee #1, a truck driver, was delivering propane to a customer. The employee parked his truck about 50 ft away and started the process of filling the fiberglass LPG tanks using the truck mounted pump and hose. Employee #1 was filling one of the last four bottles of 8 gal. tanks with the hose, when one unit, Serial Number 16626, ignited and caused a flash fire. The employee picked up the portable fire extinguisher, doused the flames out, and shut off the pump at its source. Employee #1 suffered first and second degree burns to his face, both arms and both legs. The tanks used were Lite Cylinders manufactured by a company in Tennessee, USA. The site's security personnel called Emergency Services. Employee #1 was hospitalized.
