Employee Falls from Tricycle, Injures Head

On April 29, 2007, Employee #1 was returning to his work station from the transportation office utilizing a tricycle, Serial Number 0975. He approached his work station and was turning to come to a stop, and fell off the tricycle and hit his head on the frame of the conveyor. The Tricycle remained upright and no mechanical malfunctions or damage was found to the tricycle. Employee #1 was hospitalized with head lacerations and a concussion.
