Investigation Summary

Investigation Nr: 201039062
Event: 05/11/2007
Employee's Finger Is Crushed When Window Falls

At approximately 8:15 a.m. on May 11, 2007, an employee, elementary school teacher, raised the window, 47-in. wide by 32-in. high vertical ratchet/friction type, to place a fan in it to provide cooling for the students. The heavy window had spring-activated friction tabs on each side which fit into receptacles at several spaces as the window was raised or lowered. The employee stated that this window had failed to stay at a set level several times and the maintenance department had stated it was fixed. Thinking the window was set, she let go of it to better handle the fan, leaving her right hand resting on the raised sill. The window rapidly descended, smashing her right middle finger. Her finger was crushed at the first digit, which later resulted in the amputation of a part of the first digit in order to facilitate finger reconstruction. She was hospitalized.
