Employee Amputates Finger in Machinery

At approximately 6:00 p.m. on August 25, 2006, Employee #1, a maintenance assistant, was lubricating a cam disc surface of a Simplex filler machine (Model Number AS-1, Serial Number 8085) with grease, while the machine was running. Employee #1's right index finger got caught in the machinery and was amputated. He was taken to Western Hand Orthopedics, treated, and released.
