At approximately 8:30 p.m. on May 21, 2006, Employee #1, a machine operator's helper, fractured his right elbow in seven places when he fell approximately seven feet while descending a ladder affixed to an elevated work platform. A plastic film extruding machine was mounted on the platform. Employee #1 was taken to the hospital and remained there for five days for treatment, including the reconstruction of his elbow,
