Investigation Summary
At approximately 3:45 p.m. on October 11, 2006, Employee #1, a laborer, and coworkers were loading steel pallets into a pallet magazine. The steel pallets housed the concrete unit, V3-12F Vibrapac for transportation. The concrete unit was used to compact concrete into slabs. It received concrete into a mold unit that undergoes vibration and compressive force, forming the concrete slab. During the loading of the pallet, a damaged steel pallet was inserted into the pallet magazine. The machine was powered down (turned off) and locked and tagged out, and then the damaged pallet was removed. As Employee #1 and coworkers reloaded the magazine, the machine operator restarted the machine that caused the feeder box to push forward (outward) and strike a steel pallet. The force of the feeder box ejected the pallet from the magazine, which struck the face of Employee #1. He was transported to a medical center. At the hospital, he underwent a medical evaluation and received medical attention for his facial fractures. Following the medical procedures, he was hospitalized.
