Employee Is Injured in Fall When Ladder Slips

At approximately 5:00 p.m. on October 1, 2006, Employee #1 had placed a Keller 20-ft metal extension ladder against some metal shelving. The ladder extended approximately 12-in. above the shelves. He had ascended and descended the ladder two or three times. Employee #1 was injured when the ladder slipped on the tile floor, and he and the ladder fell approximately 6 ft to 7 ft to the floor. He was hospitalized for unspecified fractures.
