Investigation Summary
On March 1, 2006, Employee #1 was installing roofing panels on a steel building that measured 18 feet high at its eave and 22 feet high at the peak. Employee #1 was located halfway between the peak of the roof and the eave, when he lost his balance and fell 20 feet to the concrete floor. Employee #1 was killed.
