Employee's Legs Are Crushed by Paper Roll Wrapping Machine

On March 24, 2008, Employee #1 was operating a paper roll wrapping machine. A kicker moved 600 lbs rolls of paper to the machine. Operators routinely entered the semi-enclosed kicker or wrapping area of the machine to clear jammed paper pieces, clear electric eyes, or to rethread paper wrap. They would turn the machine off, but did not lock it out before entering. Lock-out procedures had been developed previously, but were not followed because they had not been updated as machine control modifications were made. Electric eyes controlled the sequencing of the rolls moving into and out of the wrapper area and the wrapping operation. A piece of paper may have been covering an electric eye which may have caused the machine to stop. Employee #1 did not turn the machine off before he entered the kicker area. He was injured when the machine cycled and his legs were crushed between the kicker and a roll of paper. He was hospitalized for amputation of one or both legs.
