Employee injured in fall from ladder

On May 16, 2006, Employee #1 was using a ladder to reach an electrical junction box above an acoustic tile ceiling. He fell from the ladder and sustained injuries that required hospitalization. Through measurements taken after the accident, it was determined that Employee #1 had been standing above the highest allowable rung, as determined by the manufacturer.
