Investigation Summary
On January 4, 2007, an employee removed a buckle type clamp that secured a cap on the end of a 6 inch steel pipe riser on a sewage piping system next to the human waste lagoon at a hog facility. When the clamp was released, it allowed an unknown amount of pressure in the line to escape, causing the cap to strike the employee in the head. The employee was killed.
