Investigation Summary
On October 18, 2009, Employee #1, a maintenance man was restringing a wire making machine. He was jogging the machine through the process, when the lead wire attached to the come along used to pull the wire through the reduction die broke. The come along whipped around the bull wheel and struck Employee #1 in the head. Employee #1 died several hours later at a hospital. At the time of the accident, Employee #1 was not using the jog stick normally used on this machine to hit the jog button, which is located behind the wire. The purpose of the jog stick was to keep the worker away from the machine in the event wire through the reduction die broke.
