Employee Is Killed When Tanker Truck Explodes

On February 27, 2006, Employee #1 was on top of a tanker truck that was not grounded filling it with lube oil. When the top and bottom hatches were opened, there was a very strong odor of gasoline. The office personnel were notified, and it was pulled out of line and parked. Approximately an hour later, the company was given authorization to fill it. Approximately ten minutes into filling the tanker, an explosion occurred. Employee #1 was thrown approximately 70 to 80 ft away on top of a storage tank. He had second and third degree burns from his head to his knees and almost every bone was broken from his head to his chest. Employee #1 died.
