Employee Is Killed in Fall from Ladder

On February 24, 2006, Employee #1, a maintenance assistant, was on a 32 ft aluminum extension ladder, approximately 25 ft high. He was checking for loose wire connections on an outdoor light wall pack. The supervisor was holding the ladder, when Employee #1's packet of cigarettes fell from his pocket. The supervisor picked them up, and he heard Employee #1 yelling. He looked up and saw Employee #1 falling from the ladder. The ladder remained in place. Employee #1 was hospitalized and pronounced dead a short time later.
