Employee Is Killed When Struck by Falling Object

At approximately 3:30 p.m. on May 1, 2007, Employee #1 and a coworker, both Hispanic males who spoke very little English, were attempting to move and stack end units of steel shelving with a forklift. The coworker was moving a large steel end piece of shelving with the forklift and Employee #1 was spotting. They were attempting to stack the end units against a wall in the warehouse. The steel unit being moved struck another steel unit. This made the unsecured unit, already stacked upright, fell striking Employee #1 on the head. Employee #1 was killed.
