Employee Dies after Fall from Elevated Work Platform

At approximately 8:30 a.m. on June 19, 2006, Employees #1 and #2 were inspecting the bottom of the bridge, measuring parts for replacement. They were elevated in a basket, which was on the forks of a fork truck (Square Shooter, Model Number SS-836). The metal basket, which was approximately 48 in. wide, 72 in. long, and 42 in. high, was not attached to the fork truck. The bottom of the platform had lifting channels, but the forks were not inserted into the channels. When the two employees moved to the same side of the platform, the platform and both workers fell 25 to 30 ft to the ground. Both employees were hospitalized. Employee #2 sustained fractures. Employee #1 died seven days later.
